Information for those who are in charge of conducting their in-house Campaign.

Employee Pledge Form (PDF file)
This form can be used by the employee of an organization conducting an internal United Way campaign to make a pledge to the current United Way Campaign.

Individual Pledge Form (PDF file)
This form can be used by a retiree or other individual to make a pledge to the current United Way Campaign.

Donor Choice Form (PDF file)
This form can be used to make a donation to a specific organization associated with the current United Way Campaign.

Manley's Mighty Mart Incentive Card (PDF file)
This form can be used to sign up for the Manley's Mighty Mart incentive which is part of this year's United Way Campaign.

This form can be used to sign up for the Olum's incentive which is part of this year's United Way Campaign.
